Coronavirus 19/COVID-19 Vaccination Information

The Sacred Peaks Health Center COVID-19 Vaccine is now available Monday-Friday from 8:00 AM to 9:30 AM MST by appointment only. Call ahead to schedule your appointment at 928-283-1505. The vaccine is available to any Federally Recognized Tribal member age 16 years and older.


Who can get a COVID-19 vaccine at this event?

  • Must have a Patient Medical Record established at Tuba City Regional Health Care, Sacred Peaks Health Center, or LeChee Health Facility

What to prepare and expect?

  • If possible, please have a driver and Navajo interpreter for elderly.
  • Download and complete COVID-19 Vaccine Form
  • Must commit to return for 2nd dose of the vaccine
  • Expect to wait 2-hours or more. Bring snacks and water.

Can you get a COVID-19 Test at SPHC during the event?

No. The Sacred Peaks Health Center COVID-19 Triage Tent will be closed during the event - February 19, 2021. The Coconino County Health Department COVID-19 testing will be available.

Do not show up if you:

  • Have a cough, fever, shortness of breath, or loss of taste or smell or any other illness.
  • Had COVID-19 in the past 6 weeks.
  • Are in quarantine for a household member with COVID-19 or an exposure.
  • Have a pending COVID 19 test.
  • Had any other vaccination in the past 14 days.
