The oversight of these programs lies in one department, facilitating teamwork, a shared vision, and collective efforts. These programs work individually and collaboratively, in both the clinic and community settings, to cover the spectrum of primary, secondary, and tertiary diabetes treatment and prevention. Additionally, many partnerships with outside organizations are fostered and nurtured. We have found this integration to be critical, given the amount of work to be done within the context of limited resources. Indeed, this approach helps us provide additional resources and expertise, offer a continuum of support to patients and community members, and facilitate referral and retention across the various programs. Each program is dedicated to improving the quality of life for the people we serve and our surrounding communities.
