What is Patient Centered Medical Home? And, is TCRHCC using this model?

The Patient Centered Medical Home (PCMH) is a nationally-recognized primary health care model for children, adolescents and adults.

Tuba City Regional Health Care Corporation (TCRHCC) is moving toward using this new model of care, where we work closely with patients and their families, recognizing the unique needs, cultures and beliefs of each patient.

Your Care Team will include:

Medical Provider
Registered Nurse (RN)
Certified Medical Assistant (CMA)
Certified Nursing Assistant (CNA)
Patient Access Specialist (PAS)
Client Service Technician (CST)
and other staff who support you but the most important member of your care team is – YOU.

What are my responsibilities as a patient under the Patient Centered Medical Home model?

Follow the care plan your Care Team has developed for you.
Take all your medications as prescribed.
Attend all of your scheduled appointments.
Tell us when you don’t understand something, ask us to explain it again.
Tell us when you receive health care from other health care professionals that are not a part of your Care Team, i.e. if you go to Flagstaff Medical Center, or to a Phoenix Medical Center.
Tell us if you are experiencing any changes in your health.

What can I expect from my appointment under the Patient Centered Medical Home model?

Your team can help answer your questions and together identify your particular health care needs.
If you need to see a provider outside of Tuba City, your Care Team can support you through the process.
When you have a concern, your medical home team will work with you to find the best way to deal with them.

What are the benefits of a Patient Centered Medical Home?

You will be seen by the same Care Team during each office visits.
Your Care Team will help answer your questions.
You will have access to appointments.
You are in control of your health.
We will help you understand your health condition.

Together, you and your Medical Home Team will work on a plan that is just for you. Here is a check list to help you communicate with your Care Team:

Write down the members of your team.
Write your questions down and bring them to your appointment (it may take more than one visit to answer them all, but if you keep a list you can keep track of your questions).
Talk to your team about which health issue to work on first.
Repeat back in your own words what you understand about your care plan so you and the team know the information is clear (this may be done with any member of your team).
Ask your team how to reach them after hours.
Before leaving the clinic, know what things you need to work on to prepare for your next visit.
