Sacred Peaks Health Care Center (SPHC) is a non-emergency primary health care center with services provided by board-certified physicians and support staff within the city of Flagstaff, Arizona.

SPHC provides non-emergency services by appointment only, including some same day appointments for established patients.

How To Obtain Services

A patient must have a medical record from Tuba City Regional Health Care (TCRHCC), Sacred Peaks Health Center, or LeChee Health Facility to receive services. Indian Health Service beneficiaries who need to create a medical record with TCRHCC must bring their Certificate of Indian Blood (CIB) and insurance information.
