Coronavirus 19/COVID-19 Vaccination Information

The LeChee Health Facility is hosting a Drive-thru COVID-19 Vaccine event for adults aged anyone 16+ years and older.

  9:00 AM to 3:00 PM   9:00 AM to 3:00 PM

  - Anyone 16+ older (Minors must be accompanies by a parent or guardian)   - 16+ older (Minors must be accompanies by a parent or guardian)

 

Who can get a COVID-19 vaccine at this event?

  • Must have a Patient Medical Record established at Tuba City Regional Health Care, Sacred Peaks Health Center, or LeChee Health Facility

What to prepare and expect?

  • If possible, please have a driver and Navajo interpreter for elderly.
  • Download and complete COVID-19 Vaccine Form
  • Must commit to return for 2nd dose of the vaccine
  • Expect to wait 2-hours or more. Bring snacks and water.

IMPORTANT NOTICE

Do not show up if you:

  • Have a cough, fever, shortness of breath, or loss of taste or smell or any other illness.
  • Had COVID-19 in the past 6 weeks.
  • Are in quarantine for a household member with COVID-19 or an exposure.
  • Have a pending COVID 19 test.
  • Had any other vaccination in the past 14 days.
